This Privacy Policy explains what information we collect through this website, how we use it, and the choices you have. It applies to Pedialliance.com only and does not apply to the treatment records or protected health information PHCA maintains as part of providing medical care, which are governed by our HIPAA Notice of Privacy Practices, linked below.

Pedialliance.com and the services described on it are directed to existing and prospective patients located in Florida. PHCA does not solicit, advertise to, or seek to provide services to residents of California, and this website is not directed to, or intended for use by, California residents. If you are located in the state of California, please do not submit personal information to us through this website. Nothing in this Policy is intended to make this website subject to laws that, by their terms, apply only to businesses that direct their websites to, or collect information from, California residents.
Information You Provide Directly
You may choose to give us information directly, such as your name, address, phone number, email address, or insurance carrier, when you use a form on this site — for example, to request general information about our practice, request a tour, subscribe to our e-Update newsletter, submit a comment, contact our business office, or update an address or insurance record.

Our treatment of protected health information as a HIPAA-covered entity is described in our separate HIPAA Notice of Privacy Practices. That notice, not this Privacy Policy, governs how we use and disclose health information in the course of providing care.
