Electronic Requests:

We can now send and receive medical records electronically to patients encrypted upon request.

All medical records requests (email, faxes, and voicemails) will be handled based on priority level (one to two weeks).

  • If you are joining our practice, once we receive the patient’s complete records, a chart will be created, and the vaccine record will be preloaded and scanned into the chart.
  • If we notice any gaps in vaccine history, we will notify the parent/patient to obtain missing information.
  • Once adequate records have been received, we will call the parent/patient to schedule an appointment.

In-person requests: 

All in-person and faxed requests will be forwarded (faxed) to the central medical records fax number at (813)768–0700.

In addition to other requests: 

  • New patient wanting to establish with the practice.
  • Patient’s requesting to transfer out of the practice.
  • Court-ordered requests.
  • Medical records for special requests (i.e.- parent or patient would like a copy of medical records for specific dates of service).
