We are now partnered with Datavant, which allows for a seamless process to request new patient records and provide records to patients transferring out of the practice.

  • All medical record requests submitted will be handled based on priority level. Please allow 5 to 10 business days for processing. If you require immediate assistance, please call us directly, and we will assist you.
  • If you are joining our practice, we will schedule your new patient appointment and require a copy of the shot record in advance. Please complete the medical release form by selecting one of the links below to begin the process of obtaining records.
  • If you already have your previous records, please email them to sendrecordstophca@pedialliance.com and include your contact information.

In-Person Requests:

All in-person requests will be handled by the office. If you need a copy of your immunization record, you may download it from the patient portal or stop by one of our offices to obtain a printed copy.

If you are requesting records for your personal use, please click on the link below to submit a request to the Central Medical Records Department.
