Pre-Visit Form Completion Expectations:

As part of pre-visit planning, you may be sent forms via email and text in the days prior to your visit. Completing forms in advance is crucial as it allows our team to prepare ahead of your child’s visit, ensuring that we are as efficient and productive as possible. Form completion is not optional because it is a key part of data collection that is used by our office to provide the best and safest care for your child. If you experience any issues accessing forms or checking in, please call your PHCA office for assistance.
