APPOINTMENT SCHEDULING

We are currently scheduling appointments for COVID-19 vaccine for ages 6 months and older. 

What You Need to Know:

  • If you would like your child to receive the vaccine at a visit that is already scheduled (such as a wellness checkup), please ask at your visit.
  • For vaccine-only visits you will be asked to schedule your appointments in advance for both the first and second doses.
  • Vaccine doses will be administered inside the office in a private exam room.
  • After a dose is administered, patients will need to remain on-site for an observation period.

VACCINE TYPE

Ages 6 Months to 4 Years: Moderna

The vaccine is a primary series of two doses administered 4 weeks apart.

  • Although the Moderna vaccine is approved for ages up to 6 years, PHCA is recommending it for patients before they turn 5 so they can complete the series with the Moderna vaccine.
  • PHCA only offers the Pfizer/BioNTech vaccine for ages 5 years and older (see below).

Ages 5 Years and Older: Pfizer/BioNTech 

The vaccine is a primary series of two doses administered 21 days apart.

  • Vaccine dosages are determined by age, not a child’s size or weight.
  • Note: Some children may be 11 years old when they get their first dose and 12 at the time of their second dose. They should receive a dose based on their age on the day of vaccination, according to the CDC.

A booster is recommended for ages 12 years and older:

  • Patients ages 12-17 years can receive a booster of the Pfizer-BioNTech vaccine only.
  • Patients ages 18 years and older can receive a booster of either Pfizer-BioNTech or Moderna vaccine. (please note PHCA only offers Pfizer-BioNTech)
  • The booster should occur 5 months after completing the primary vaccine series.
